Student Retention Alert Form

You will need to login with your UNH ID to gain access. Use "UNH\your username here" (no quotes) as your login. There are a number of searches available. This utility only functions inside the University network at this time.

Please submit this form for students that you come into contact with and who share any issues/problems that they are having related to their UNH experience with you. Note this form is to be used for all students, both undergraduate and graduate students. When completing the form please enter the student's first and last name. Any additional information you can enter to help us identify the student is helpful, but not necessary. You also do not need to enter the student's ID number if you do not know it.
