Step 1. Register with Campus Access Services by completing a CAS Intake Form and submitting appropriate documentation of a disability. Click here to be directed to the Intake Form. Additional forms are available on this link to assist you with providing appropriate documentation as well as a link to documentation guidelines.
New Students entering the university for the Fall semester should submit their Intake Packet by JULY 15 to ensure enough time to determine the student's eligibility and the appropriate reasonable accommodations. If accepted after July 15, students should submit the Intake form and documentation as soon as possible, but not later than August 1. Students entering the university for the Spring semester should submit an Intake Form and documentation as soon as they are accepted to the university, but not later than JANUARY 10. Students submitting documentation after these dates may experience a delay in receiving reasonable accommodations.
Documentation will be reviewed to determine the student’s eligibility for reasonable accommodations under the Americans with Disabilities Act Amendment Act and/or Section 504 of the Rehabilitation Act, Subpart E.
Step 2. Meet with a Campus Access Services staff member to complete an Intake Interview.
- Intake interviews are offered both in the summer (between June 1 and July 31) and at the start of school. Students needing specialized services such as alternate format textbooks, ASL interpreters/captionists, notetakers, FM Systems or other auxiliary support services should schedule appointments in the summer to enable the university to mobilize appropriate resources and assure the timeliness of the provision of accommodation. The Intake Form and appropriate documentation must be received and reviewed before an appointment can be scheduled.
Step 3. Registered students are strongly encouraged to attend D.R.E.A.M. Orientation where they will learn the significant differences between their responsibilities as a student with a disability in high school versus being a student with a disability in college. They will be informed about services and resources available to make their transition from high school to the university community a smooth process.
Students attending DREAM Orientation who have not already completed an Intake Interview will automatically be scheduled for an intake appointment to complete required paperwork and discuss the reasonable accommodations they are eligible to receive at the postsecondary level. As an alternative, students who have not completed an Intake Interview who also cannot attend D.R.E.A.M. Orientation should make an appointment to meet with a Campus Access Services staff member for an interview at the start of the semester.
