Reporting Requirements while on OPT

All students on OPT or STEM OPT must report to the International Services Office (in person, by e-mail, or by phone) within 10 days if any of the following change:

  • Legal Name
  • Residential or Mailing Address
  • Employer Name
  • Employer Address
  • Status of Employment (including termination, promotion, etc.)


Failure to report these changes in a timely manner can result in termination of your work authorization period and may result in ineligibility for further work authorization applications.


Additionally, students on STEM OPT must comply with the following reporting requirements:

Six-Month Validation: Every six months students must work with their DSO to confirm accurate reporting in their SEVIS records by providing the following:

  • Legal Name
  • Residential or Mailing Address
  • Employer Name and Address
  • Status of Current Employment


Failure to provide this validation reporting in a timely manner can result in termination of your work authorization period and may result in ineligibility for further work authorization applications.

Annual Self-Evaluation: Each STEM OPT student is required to submit to their DSO an annual self-evaluation about the progress of their training experience.

If a student remains with the same employer during the duration of their employment, they must submit their "Evaluation On Student Progress" after 12 months with their employer. Students must also submit their "Final Evaluation On Student Progress" upon completion of their 24 months of STEM authorization. Both sections can be found on pg. 5 of the Form I-983.

If a student changes employer during their STEM OPT period they must submit the "Final Evaluation On Student Progress" upon termination of their employment with the company. This section can be found on pg. 5 of the Form I-983.

Failure to provide the Annual Self-Evaluations in a timely manner can result in termination of your work authorization period and may result in ineligibility for further work authorization applications.
